• Pre-sell frozen pizza certificates; seller delivers all certificates to purchasers.
• Players earn 38% back on sales ($5 a pizza).
• Each coupon is good for one 9” frozen Deep Dish pizza (cheese, sausage, pepperoni, or spinach ONLY) to be picked up by customers at any Lou Malnati’s location, including Arizona, Indiana, and Wisconsin. Good for 6 months. The expiration date will be at the bottom of the coupon. The gluten-free option is excluded.
• Complete the order form and return it by October 11th.
• One check must be made to the Northwest Chargers for the entire order amount.
